There is a popular misconception in many Safety communities, that ‘RCA’ approaches are contradictory versus complementary to ‘Learning Teams’. What is the basis for this misconception? Most every investigative occupation follows the same process steps, so we should be focusing on the steps and not the labels.

Many characterize all ‘RCA’ as having the equivalent simplicity of conducting a basic 5-Why analysis? Why? Because that is NOT the reality in the field. We will discuss what are the potential motivations to put ‘RCA’ in that box’?

When looking at ‘RCA Differently’, we realize that many holistic RCA approaches have been incorporating learning team/human reliability principles for decades. We will explore the potential synergies that exist between these two tools, despite their perceived differences.

The presenter will be a 35+ year RCA veteran who joined CHOLearning because of how their perspectives on human and organizational systems, enhanced his own RCA approaches in the field. This will be a highly interactive session with a townhall style format for constructive discussions. Most importantly, you’re going to have FUN!
